This posting will be used to establish a 12-month eligibility list which will be used to fill future permanent, term, AOC and casual vacancies within the Continuing Care branch. This job is based in the thriving and diverse city of Whitehorse in the beautiful Yukon.

Your role
The Social Worker provides professional social work services to clients in Home Care within the scope of practice, standards of practice, organizational policies and procedures, and competencies.
The responsibilities for this position include working in an interdisciplinary team environment to provide psychosocial assessments and social work services for all clients and their families, including and providing care coordination, support, responding to issues of abuse, neglect & self-neglect, advocacy services and providing education & support to palliative and complex clients and their families for the development of Advanced Directives and end of life planning. You will participate in the development and implementation of programs and perform other related duties.

Essential qualifications

- Bachelor of Social Work
- Experience providing case management as a Social Worker
- Experience completing assessments

Conditions of employment
You must meet the following conditions before being hired, or within the time frame noted.
TB Screening, Vulnerable Sector RCMP security clearance, CPR Level C with AED, valid Class 5 Yukon Driver’s License, WHMIS 2015 and current registration with a Canadian Social Work regulatory body
Job requirements
This job requires you to perform, and/or be exposed to, the following:
Exposure to pets, exposure to second-hand smoke, latex products, flowers and/or plants and other irritants while in client homes or within facilities. Travel within Whitehorse and surrounding areas is required
