This job is based in the thriving and diverse city of Whitehorse in the beautiful Yukon.
Who we are
The Referred Care Clinic is a primary care clinic for vulnerable adults with unstable mental health and/or substance use disorders. The RCC also operates alongside Opioid Treatment Services which supports any resident wishing to seek help in managing their opioid use. The RCC is a multidisciplinary team composed of outreach support workers, licensed practical nurses, registered nurses, nurse practitioners, physicians, social workers and medical office assistants. The Referred Care Clinic is embedded within the Mental Wellness and Substance Use Services (MWSU) branch of government, and we work closely and collaboratively with all of MWSU, including direct support in the Withdrawal Management program.
Your role
Reporting to the Clinic Supervisor of the Referred Care Clinic, this position is responsible for conducting physical and mental health assessments, and developing and implementing treatment plans, case management, including physical and mental health counselling/education to clients with mental illnesses and/or substance use disorder. Services are largely delivered through home visits and community outreach, with occasional appointments held at the clinic or satellite pop up locations.
Community Mental Health Nurses (CMHN) work autonomously and in collaboration with others to support clients and their natural supports to achieve optimal health. CMHNs deliver primary health care and mental health services, provide education, leadership, coordinate care, and support clients to manage their own health. CMHNs function in a collaborative practice model as part of an interdisciplinary team to provide comprehensive primary and/or mental health care services to people living in the Yukon with significant mental health and/or substance use issues.

Essential qualifications
Please submit your resume clearly demonstrating how you meet the qualifications below. Selections for further consideration will be based solely on the information you provide in your resume.
- Verification of post-secondary Bachelors degree in either Psychiatric Nursing or Nursing, OR Diploma in Psychiatric Nursing with considerable, recent, and relevant experience performing medical procedures (i.e. as phlebotomy, etc.) in a mental health environment;
- Eligible for Registered Nurse registration with a Canadian registered nursing regulatory authority or a Registered Psychiatric Nurse licencing with PLRA;
- Recent experience in providing mental health assessments, nursing interventions, case management, and support services to clients with a range of mental health and substance use presentations (i.e., schizophrenia, bipolar disorder, depression, anxiety, substance use concerns, interpersonal conflicts, etc.);
- Recent experience completing suicide risk assessment and creating safety plans;
- Experience providing psychoeducation to clients, families, and support staff;
- Experience providing physical health assessments, and chronic disease management.

- Able to provide services related to harm reduction and/or knowledge of other services designed to meet the needs of people who use substances, including Opioid Agonist Therapy services

Conditions of employment
You must meet the following conditions before being hired, or within the time frame noted.
- Verification of Degree in Nursing or Psychiatric Nursing OR Diploma in Psychiatric Nursing
- Registered Nurse registration with the Yukon Registered Nurses Association OR Registered Psychiatric Nurse licencing with PLRA
- Current TB Screening;
- Current RCMP Vulnerable Sector Clearance
- Valid Class 5 Driver’s Licence or equivalent
- CPR C & AED; or, Health Care Provider AED; or, BLS with oxygen;
- WHMIS (within one month of hire)
Job requirements
This job requires you to perform, and/or be exposed to, the following:
- May be exposed (to clients with communicable disease symptoms (e.g., coughing, GI issues), bodily fluids, tissue samples, or active bleeding.
- May encounter household hazards during outreach/home visits, including weapons, aggressive individuals, pets, allergens, second-hand smoke, drug paraphernalia, bodily fluids/waste, and infestations (e.g., bed bugs, lice).
- May work with clients exhibiting unpredictable or risky behaviour, including those posing a risk to themselves or others.
- Clients may present with deteriorating mental status or heightened emotional states that could require RCMP involvement.
- Role involves physical demands (reaching, bending, lifting, awkward postures), supporting client transfers (up to ~150 lbs partial weight), and carrying equipment up to 15 kg; occasional latex exposure possible.
- Daily travel in all weather conditions may be required; itinerant work away from home for several days at a time is less frequent but also a part of this position.
- Outreach care may occur in environments associated with drug use or sex work.
- Potential for verbal, emotional, or physical aggression from reactive patients.
