Our team
The Yukon Palliative Care Resource Team (PCRT) is one part of a collaborative system of care providers offering specific services to people with life-limiting illnesses including their family and caregivers.
The primary focus of this position is to provide specialized clinical Social Work practice leadership and teaching in order to build capacity in palliative care within the Yukon Territory. The successful candidate will be part of a multi-disciplinary palliative clinical resource team and practice collaboratively with other members of the interdisciplinary team to support the philosophy, vision and mission of Hospice Palliative Care and the delivery of quality consistent care. As a member of the Palliative Care Resource Team (PCRT) this position is a clinical resource to acute care, residential care and the community

The Palliative Care Program is looking for a dynamic, organized registered professional with strong clinical and teaching skills who is passionate about mentoring others and developing capacity in Hospice Palliative Care. Essential qualifications
Please submit your resume clearly demonstrating how you meet the qualifications below. Selections for further consideration will be based solely on the information you provide in your resume.
- Bachelor of Social Work
- Current registration or eligible to be registered with a Canadian Social Worker regulatory body
- Experience practicing as a social worker in a healthcare environment
- Experience facilitating adult education sessions
- Experience providing palliative care and / or advance care planning support to both clients and families as well as other health care providers is considered an asset
- Advanced practice education in palliative care social work is considered an asset

Conditions of employment
You must meet the following conditions before being hired, or within the time frame noted.
- RCMP Vulnerable Sector Check
- TB Screening
- CPR Level C with AED (completed within the last 30 months)
- Current registration with a Canadian Social Worker regulatory body
- A valid class 5 driver’s license
Job requirements
This job requires you to perform, and/or be exposed to, the following:
- Exposure to pets, second-hand smoke, latex products, flowers and/or plants and other irritants while in client homes or within facilities.
- Occasional travel to communities is required
